What happens after you make a report:

The Office for the Prevention of Harassment and Discrimination (“OPHD”) team will review your report to determine if the conduct is covered by the UC Sexual Violence and Sexual Harassment policy or the UC Discrimination, Harassment, and Affirmative Action in the Workplace policy. If it does, we will move forward with next steps. If not, we will contact you and direct you to the appropriate UCSF office or resource.

OPHD will contact the person reported to have experienced harassment or discrimination and invite them to meet with our office and share their experiences or concerns. The impacted person can choose whether or not to respond to our outreach. If they do so, OPHD will assess their complaint, provide information about supportive resources, and offer options for next steps.


Why report:

We encourage all members of the UCSF community—patients, staff, faculty, and students—to report incidents involving sexual violence, sexual harassment, discrimination, and harassment to OPHD. Reporting helps us offer support and ensure that all members of the UCSF community have a safe and inclusive learning and working environment.


How to Report:

If you or someone you know has experienced sexual violence, sexual harassment, or discrimination and harassment based on a protected category, we encourage you report. Reports can be filed by:

To file a report anonymously, you can:

  • File an anonymous online complaint by going to the EthicsPoint website.
    • Choose “Report a Concern Online”
    • Enter a campus location for “Where Did This Occur?”
    • Read the important reminder about passwords and hit” Continue.”
    • Next select "Discrimination/Harassment" or “Sexual Misconduct” for the Topic Which Best Describes Your Concern.
    • You will need to agree to the terms and conditions and choose a password before submitting the Report.
  • File an anonymous phone complaint by calling the EthicsPoint Hotline at (800) 403-4744.

Your anonymous report will be routed to OPHD.


What to include in your report:

We recommend filing a report as soon as possible after the incident occurs to preserve the accuracy of information. Reports should include the following details (when known):

  • The name of the impacted person
  • The name of the person who engaged in misconduct
  • Your contact information and affiliation
  • Information about the prohibited conduct, including when, where, and what occurred
  • Any identities that harassment or discrimination occurred based on (i.e. race, gender, religion, disability, etc.)

  • Faculty and Staff Assistance Program (FSAP) provides confidential counseling services for personal and work-related issues to faculty and staff. Support is offered for a wide range of issues including work related stress, relationship/couple issues, family problems, depression, anxiety, or other emotional problems, grief and loss, alcohol, drug and other addictions, consultations and training services for faculty and staff management. FSAP can be reached at (415) 476-8279 or [email protected].
  • The Office of the Ombuds serves as a resource for all members of the UCSF community (faculty, non-faculty academics, staff, administrators, students, post-doctoral fellows and other trainees) and offers a safe, confidential place to discuss UCSF-related issues and explore possibilities for informally addressing concerns. The Ombuds Office can be reached at (415) 502-9600.
  • Campus Advocacy, Resources and Education (CARE) provides free, confidential support to any UCSF affiliate including students, staff, faculty, post docs and researchers who have experienced discrimination, harassment, or abuse related to race, ethnicity, national origin, or other protected categories and to any UCSF affiliate who has experienced interpersonal violence such as sexual assault, dating/intimate partner violence, sexual harassment, or stalking. You may speak to both Kendra and Denise confidentially about your experiences and receive emotional support and resources.
    • Kendra Hypolite, CARE Advocate for Racial Justice, provides confidential support, assistance, and advocacy on behalf of UCSF community members who are experiencing discrimination or harassment based on their race, ethnicity and/or national origin, and other protected categories.     • Denise Caramagno, Confidential CARE Advocate, provides confidential support, assistance and advocacy on behalf of the UCSF community members who have experienced interpersonal violence such as sexual assault, dating/intimate partner violence, sexual harassment, or stalking. For emergencies/reporting to the police

Please know that we do not provide emergency services. If you are experiencing an emergency or need immediate assistance with a matter of sexual assault, stalking, relationship violence, or other crimes, please contact the UC San Francisco Police Department by calling 911 or (415) 476-6911.

Note: A lack of "hard evidence" or significant time lapse since an incident occurred should not discourage you from filing a police report but may limit what actions can be taken. However, if possible, keep any evidence, including clothes and bedding, photographs, written notes, texts, and documents—these do not need to be submitted with the initial report.
